REFERRAL INFO
More than 50% of our patients are referred by their primary care physician or a specialist.

We offer three types of patient-based services:

Multi-week medical programs, (insurance reimbursable in most cases)
Individual psychotherapy and medical consultations (reimbursable) and education consults (self-pay)
Wellness programs and services (self-pay)


You can refer a patient to the Mind/Body Medical Institute by providing them with M/BMI materials and contact information. In this instance, the patient will initiate contact with us and usually we will request from you a signature on this referral form .

If your patient expresses a readiness to begin M/BMI services during your office visit, you may, with the patient's permission, fax the referral form directly to us, and we will contact the patient to begin the intake process.

Billing Information

If the patient's insurance plan requires a referral for outpatient services, she/he will need to request one from her/his primary physician. We will then communicate with the MD's office to obtain the referral.

The weekly programs include one initial assessment, 10 or 12 weekly group sessions, and one discharge visit. In most cases, billable claims are submitted directly to the patient's insurance carrier as medical office visits. She/he will be responsible for a co-payment at the time of each visit if the plan requires one. There is a non-refundable self-pay fee, collected at the first weekly session, to cover the cost of sevices that are not billable to insurance. Please note that insurance coverage and reimbursement may vary.

In the rare instance where reimbursement is not available, the patient will be responsible for the cost of the program. Our office staff is available to assist patients with billing and reimbursement matters.
