| "We are proud to be an affiliate of the M/BMI and to provide the outstanding programs which consistently achieve excellent patient satisfaction scores. And most importantly, the mind/body philosophy has infused our culture and influenced our patient treatment." | Rebecca Gudorf, MS Director M/BMI at St. Joseph's Regional Medical Center South Bend, Indiana |

1. Can you ensure protection from direct competition with neighboring hospitals?
2. How will M/BMI affiliation benefit our hospital?
3. Will this be a financially profitable endeavor?
4. What services do you provide on an ongoing basis?
5. Can we rely on your support for the grand opening of our M/BMI?
6. Can we offer clinical programs not already developed by you?
7. How do we perform collaborative research?
8. What start-up costs can we anticipate?
9. What are our options when the five-year contract expires?
10. Can we offer your school-based program as well?

Affiliates receive exclusivity of territory. The M/BMI would not approve another affiliate in your primary patient care area, which is usually designated by county.

An M/BMI affiliation differentiates your hospital from others in the area. Patients are actively seeking mind/body interventions, and, thus, your hospital will attract many new patients. Also, becoming an affiliate gives you all the necessary training and materials to open quickly. We consider it a "turn-key operation."

In our experience, affiliates tend to break even financially.

We assist you with clinical, administrative, marketing, research, and reimbursement matters. We provide regular teleconferences and yearly retreats for affiliates.

Absolutely. We will work closely with you prior to the opening on marketing, public relations, and other related matters. Dr. Benson and other staff are available for kick-off events.

We encourage and assist you in such endeavors. For example, we have helped affiliates launch mind/body programs for substance abuse and weight reduction.

We work from a unified database; you will collect data from your patients and send it to us in Boston. Individual research undertaken by an affiliate must be approved by our research director in advance.

Start-up costs should not be great. Usually existing staff is reallocated rather than new staff hired. Minor equipment purchases and some space renovation may be necessary.

We look for long-term continuing relationships. Renewal contracts at reduced fees have been developed for this purpose.

The Education Initiative is not part of the basic affiliate contract, but a package, which includes training and materials, is available to all affiliates for a separate fee.
