PROGRAM AND BILLING INFORMATION
The Mind/Body Program for Infertility meets Mondays from 5:30 pm - 8:00 pm. To participate in this program, you must have seen your physician or specialist for infertility treatment within the past six months.

If your insurance plan requires a referral for outpatient services, you will need to request one from your primary physician. We will then communicate with your MD's office to obtain the referral.

The program includes one initial assessment, 10 weekly group sessions, and one discharge visit. In most cases, billable claims are submitted directly to your insurance carrier as medical office visits. You will be responsible for a co-payment at the time of each visit if your plan requires one. There is a non-refundable self-pay fee, collected at the first weekly session, to cover the cost of sevices that are not billable to insurance. Please note that insurance coverage and reimbursement may vary.

In the rare instance where reimbursement is not available, the patient will be responsible for the cost of the program. Our office staff is available to assist you with billing and reimbursement matters.
